1. A lot of places. Some cheaper than others. Schwab, Scottrade, thinkorswim ( very good! ) or mypersonal favorite at the moment, interactivebrokers.
2. well, i suppose you could buy a seat on an options exchange. other than that, though, you will always have to use at least one middle man.
3. You can't. Well you can always negotiate contracts OTC (over the counter) but I suppose you are not regulated to do so.
